Paramedics wheel seventeen-year-old Luke Serone into the emergency room. His mother, Stephanie, tells Grad that Luke is allergic to penicillin and slightly asthmatic. Doctors set about their work and Stephanie makes her way to the waiting area. Watters notices Stephanie and cautiously approaches her. Though there is a familiarity between the pair, they are both weary of one another. Stephanie explains that her son was in a snowmobiling accident, but will recuperate once he undergoes surgery. Watters offers assistance when Stephanie expresses fears her insurance may not cover the accident. Attempting to keep their relationship strictly professional, Stephanie rejects his help. Later, Watters tells Luke that he met his mother many years earlier, when Stephanie was still the proprietor of a book shop. An angry Stephanie later chastises Watters for speaking to Luke without her permission.

Grad asks Austin for a consult when a pregnant woman, Leslie Stark, complains of a persistent cough and fatigue. When Austin detects signs of a heart murmur, Leslie's husband, Greg, interrupts the conversation, insisting his wife won't even take aspirin for a headache. Austin orders a series of lab tests before making a diagnosis.

Grad notices a woman wandering around the emergency room. The stranger Christine Druckhammer, claims she is several months pregnant and has been experiencing abdominal cramps... even though she has never seen an obstetrician. Grad orders an ultrasound and a pregnancy test.

Lab tests reveal that Leslie Stark suffers from a leaking heart valve, a fairly uncommon condition for someone so young. Leslie admits she consumed diet pills during her pregnancy in an effort to keep thin. This comes as a shock to Greg, who knew nothing of the drug. Austin suspects Leslie's condition may be directly connected to the pills, and hopes that heart medication will reverse its affects.

Grad tells Christine she is not, in fact, pregnant. A lab test shows an ovarian cyst called a teratoma is growing inside her body. Surprisingly, Christine refuses to grant permission for the cyst to be removed — and insists she is pregnant. When Grad and Wilkes cannot change her mind, they ask Shutt for a psychiatric consult. Shutt decides to concoct a story of his won. He tells Christine that her infant died and must be removed. Christine accepts this explanation and agrees to have the operation.

Watters tells Shutt he met Stephanie six years earlier — one year before his wife Rose died. He felt a great responsibility to Rose not to act upon his feelings. Shutt convinces him not to let his "second chance" slip through his fingers. Watters approaches Stephanie at her home, hoping to open a dialogue, but Stephanie sends him away. Later, both Watters and Stephanie find themselves in Luke's hospital room. Rather than explain the acrimony to Luke, the pair act as if they never bumped into one another earlier. Stephanie finds herself softening towards Watters. Later, the pair share a kiss.

When Leslie's lungs fill with fluid, and her heart begins to fail, Austin opts for surgery — even though it places the baby's life at risk. As the operation gets under way, the baby's vital signs begin to fade. An anesthesiologist orders an emergency C-section, but Austin overrules his decision and proceeds with the surgery despite the risk. Austin successfully completes the procedure, saving the lives of mother and child.

Meanwhile, McNeil cancels his date with Wilder after sparks fly between he and Catera. But Catera insists they become friends before any romance develops between them.
