In Short: McNeil doesn't cope well with the demands for attention from Karen, his "roommate" and a therapy patient claiming that McNeil's his father. Billy learns that Aaron kissed Diane, and handles it very badly. A depressed Nyland gets one more unwelcome surprise. Phillip receives an offer from Emma.

Grad tells Kronk that she and Shutt shared a mutual kiss. Kronk attempts unsuccessfully to hide his anger. Watters informs Austin and Shutt that Dr. Schapira has resigned as Chief of Surgery. As a result, they have both been nominated as candidates to fill the position. Wilder introduces McNeil to one of her patients, sixteen-year-old named Luke Sarison. McNeil takes an immediate dislike to the boy and heads off in another direction. Shutt asks Kronk if he can count on his support for his bid to become the next Chief of Surgery. Kronk reveals that Diane told him about the kiss, and threatens physical violence but manages to control his anger. Grad becomes incredulous when he learns Kronk threatened Shutt. Shortly thereafter, Watters assigns them both to E.R. duty. Emma tells Watters she received a promotion and will be relocating to Paris. She asks Watters to join her. A cop approaches Nyland with a warrant for the murder of Marcus Jessup. Nyland is placed in handcuffs and lead away. Later, Wilkes bails Nyland out of jail. Kronk tells Nyland that Wilkes has transferred him to work at the blood bank. Nyland is deeply offended. Shutt tells Austin that the hospital board endorsed him, but notes that the final vote isn't until that afternoon. Later, Watters tells Austin that the Board's endorsement had nothing to do with gender; the real truth is the Board simply does not like Austin. McNeil is taken aback when Luke begins behaving as if he is his father. He arranges for the hospital psychologist, Stadson, to speak with the boy. Stadson tells McNeil that Luke is coherent and exhibits no signs of delusions. Since the boy's family died tragically, Luke is projecting his deceased father onto McNeil. While enjoying a romantic candlelit dinner, Emma tells Watters she is leaving for Paris the next morning. Watters is taken aback by the suddenness of the change. Emma tells him she will wait at the airport for him; if he doesn't show up, it will mean he is not moving to Paris. Watters tells Kronk that part of the trade-off in getting married is being driven crazy by one's spouse. But he points out that, for the most part, his wife made him extremely happy. Nyland tells Wilkes it will be impossible for him to clear his name. He pays him back the bail money, then states that he has decided to leave the hospital for good. Shutt tells Austin he is declining the Chief of Surgery position. He believes Austin knows how to get things accomplished better than anyone at the hospital including himself. Wilkes relieves Grad and Kronk from E.R. duty. Afterward, Kronk apologizes for being "a jerk." Their differences reconciled, Grad and Kronk kiss. At the airport, Emma resigns herself to the fact that Watters is not coming with her to Paris. She grabs her bag and passes through a metal detector. Suddenly, Watters calls out her name. Watters thanks Emma for her company and for reminding him of what is truly important in life. Unable to kiss because of security restrictions, they say their last good-byes. Wilder cannot fathom why McNeil refuses to let anyone Luke, Harriet, or even herself get close to him. McNeil promises he will be more open. A crowd gathers in the hospital lobby for a re-dedication. Watters toasts the new owners of Chicago Hope.
Dr. Stadson to Phillip Watters:
"Isn't there a single well-adjusted,
psychologically fit person in this hospital?!"
